HousingWe assist with interviewing nursing homes, and assisted living facilities. We provide suggestions for relocation, as well as advice.

Assisted Living Facilities- Many have month to month rental agreements and can provide moderate to luxurious senior living. There are various contractual agreements.
Continuing Care Retirement Communities (CCRC) provide services such that a resident can age in place. Typically, these communities have independent living, assisted living and skilled nursing buildings all on one campus. Some even have Alzheimer's units. A CCRC provides for all the physical and psychological needs of their aging residents.
Skilled nursing centers will provide 24 /7 nursing care for your loved one. These homes vary a lot in what they offer; short-term rehab, long term rehab, physical therapy, and the overall look and feel of the facility. |
HomecareWe identify homecare companies that have the highest quality nurses and best fit for your loved one.
Homecare is divided into 2 categories- skilled care which provides for medical needs. Secondly, custodial care includes non-medical help; such as bathing, dressing and feeding your loved one. There are many choices in the Pioneer Valley for homecare. Some companies are affiliated with a hospital, but many are private companies. Homecare is typically paid for by Medicare after a qualifying hospital stay or privately by the family. |
LiaisonWe act as a liaison with physicians, eldercare attorneys, social workers, and home and personal care companies.

We work with personnel from homecare agencies to ensure they are meeting all goals laid out in your loved one's Plan of Care. Homecare aides can be individually interviewed for you to find the best match. Sensible Senior Solutions makes sure the agency offers the specific services you need such as physical and occupational therapy. We insure that the agency had the specific disease management program tailored to the senior's needs. Some of these include: orthopedic rehab or wound care programs. |

Levels of Services Offered Include: Tier 1 - Initial consultation and recommendations that meet your families goals.
Tier 2 - Recommendations as well as assistance with appointments and decision making.
Tier 3 - Tier 1 and 2 service along with assistance up to and including placement of your loved one in an appropriate setting.
Tier 4 - Ongoing management of your relative's changing needs. Services include helping plan appointments and meeting with prospective providers and caregivers. |
